   4. An itinerary package mailed prior to each project to include airline and hotel confirmation numbers, hotel and facility phone numbers, directions to facility, contact information for The MSR Group, etc.
   5. Miscellaneous perks.

1. Annual membership fee of $1,200 entitles moderator to our concierge services twelve (12) months beginning the month the fee is paid. Concierge services include all bids, coordination and scheduling of focus groups and travel arrangements, as well as all amenities we offer.
2. A rebate of $100 will be applied to the final invoices of first twelve (12) completed and billed projects during each membership year.
3. Invoicing for the membership fee occurs at the time of sign-up. No work will begin until we receive payment.
4. To participate in the program, each moderator must provide a credit card which will be kept on file for travel arrangements and any other costs the moderator wishes.
5. Upon booking a focus group project, The MSR Group will oversee all recruiting and details of facility rental for a 20% markup of all expenses (e.g. recruiting and facility rental charges). For all direct bill services (i.e. transcriptions and recruiting conducted in-house by The MSR Group) no mark up is applied.
6. For each project, the respondent co-op invoice will be sent immediately upon booking and a final invoice will be sent within 30 days of project completion.
